Please take a few minutes to complete a "Testimonial" form.  We also are planning to include "testimonial spots" on our website.  Just download, print and complete the form and return it to Jay Rosenthal, 253 Lowell Ave. N.E., Warren, OH 44483.

Q.  What is a testimonial?

A.  A testimonial or endorsement is a written or spoken statement, sometimes from a public figure, sometimes from a private citizen, extolling the virtue of some product, which is used in the promotion and advertising of that product.

 Q.  Why should I give a testimonial?

A.  CISV is not just an opportunity for (summertime) travel,  it is a way of life.  Only if you believe this should you consider giving a testimonial so that others who don’t know about CISV or have questions about the value of such an experience may know from someone who has already had the experience.

Q.  Which form should I use?

A.  There are four forms:

  1. “Traveler’s” Testimonial – Youth  (WORD)  (PDF)

Should be used by anyone who traveled and is currently under 18 years of age.  A parental signature is required.

  1. “Traveler’s” Testimonial – Adult  (WORD)   (PDF)

Should be used by anyone who traveled and is currently 18 years of age or older.

  1. “Know a Traveler” Testimonial – Adult  (WORD)   (PDF)

Should be used by anyone who is reporting on someone who traveled and is currently over 17 years of age.
